Terms of service

These Terms govern your use of Patient Care Circle ("PCC", "the service"), operated by [Patient Care Circle]. By creating an account you agree to them.

What the service is — and isn't

PCC is a secure coordination layer for sharing messages and documents among a patient's care team. It is not an electronic health record, not a medical device, and does not provide medical advice. It is not for emergencies — if you have a medical emergency, call 911.

Accounts & eligibility

  • You must provide accurate information and keep your credentials secure; you're responsible for activity under your account.
  • Clinician accounts must complete license verification before accessing patient information; verification gates access and does not, by itself, confer it.
  • Patients control their own circle and consents; access is governed by our authorization model.

Acceptable use

  • Use the service only for lawful, authorized care coordination.
  • Don't attempt to access data you aren't authorized to see, disrupt the service, or reverse-engineer it.
  • Use the service only for lawful, authorized care coordination. Protected health information is handled under our executed Business Associate Agreements and security controls.

Plans & billing

We offer Free, Professional, Agency, and Enterprise plans as described on our pricing section. Paid plans are billed monthly through our payment processor (Stripe); you can cancel at any time and retain access through the end of the paid period. Fees are non-refundable except where required by law. We may change pricing with notice; changes don't affect the current billing period.

Your data

You retain ownership of the content you submit. We process it to provide the service under our privacy practices and, where applicable, a Business Associate Agreement. You can export and delete content as described in the product.

Disclaimers & liability

The service is provided "as is" without warranties of any kind. To the maximum extent permitted by law, PCC is not liable for indirect, incidental, or consequential damages, and our aggregate liability is limited as set out in the final agreement. Nothing here limits liability that cannot be limited by law.

Termination & changes

You may stop using the service at any time; we may suspend or terminate accounts that violate these Terms. We may update these Terms and will post the effective date above. Continued use after an update means you accept it. These Terms are governed by the laws of [governing-law jurisdiction].
